Post-Incident Report Incident: Physical Host Outage — Partial Service Interruption Date: 31 October 2025 Region: West Status: Resolved

At 05:13 AEST (03:13 AWST), monitoring detected that a physical host in the West region became unreachable. Engineers investigated immediately and found the system fully unresponsive to standard management access. A controlled out-of-band power cycle was initiated, restoring the host to service at 05:43 AEST (03:43 AWST).

After recovery, testing revealed a routing table entry had not reloaded correctly, delaying full restoration until 06:52 AEST (04:52 AWST). All services were confirmed stable and available by 07:18 AEST (05:18 AWST).

Impact was limited to a small number of services hosted in the West region. Other regions and core systems were unaffected.
